Adult Mental Health

Case Management - Rule 79  – This voluntary service offers assistance to seriously and persistently mentally ill adults in obtaining necessary mental health social services, financial assistance, 医疗, 法律, vocational, 住房, and other services designed to facilitate their adaptive community adjustment. Services may include referral assistance; payment; transportation; 72-hour hold: adult acute care hospital involuntary inpatient involvement and payment; Pre-petition Screening / Hearing; placement in a Community Behavioral Health Hospital (CBHH) and/or Intensive Residential Treatment Service (IRTS) or referral to the Southwinds Assertive Community Treatment (ACT) Team for services. Click here for more information.

Community Support Program (CSP) - This is a voluntary program to help people manage their mental health symptoms, participate in their community, learn medication management and develop daily living skills. Funding is from a DVHHS grant through individual service arrangements with the provider to work with clients of DVHHS. Click here for more information.

Children's Mental Health

Children’s Mental Health Case Management is a service provided to assist families in: determining if any mental health problems exist, determining what types of treatment or services can help deal with these issues, arranging necessary services, making follow-up contact to ensure that needed treatment is being provided.

Case management services are voluntary and include assisting the family in obtaining a comprehensive diagnostic assessment, 如果需要, developing a functional assessment, developing an individual family community support plan, and assisting the child and the child’s family in obtaining needed services by coordination with other agencies and assuring continuity of care. Case managers must assess and reassess the delivery, appropriateness, and effectiveness of services over time.
